Appointments can be made by calling or texting the office phone.  Because our office utilizes secure electronic health records, once your appointment is scheduled, you will receive a link to complete intake information prior to your appointment.  When you tap submit, your information goes directly into our computer system!  Upon your arrival, insurance cards will be scanned, and you will be asked to use the iPad to provide information about your current condition.
